Our Mission: Austin Smiles – The Austin Plastic Surgery Foundation is a nonprofit organization whose mission is to provide sustainable surgical care to children born with cleft lip and cleft palate both in Central Texas and Latin America.

Thank you for your interest in joining Austin Smiles Board of Directors! We would like to learn more about you. Please provide the following information about yourself, to ensure the best match between you and Austin Smiles. This form can be accompanied by a current resume or any other useful documentation that is appropriate for the Board to review.

If you are chosen to serve on the Austin Smiles Board of Directors, you will agree to the
following: provide at least 2-4 hours a month in attendance of board meetings, committee
meetings or volunteering; annually donating or helping fundraise a minimum of $1500 to Austin
Smiles to support the sustainability of the organization; you do not have any conflict-of-interest
in participating on the Board.
